wisdom Tooth Pain

Wisdom tooth pain is common. Most people do not have room for their wisdom teeth and this limited space causes many wisdom teeth to flip into unusual positions. Because of the very high incidence of problems, it is usually a very good idea to have this tooth removed early in life.

Partially erupted wisdom teeth often have gum pouches over their tooth crowns that can get infected and cause discomfort. A poorly positioned wisdom tooth can improperly press against the second molar in front of it and trap food. This food trapping causes tooth decay and wisdom tooth infection . In some cases, the wisdom tooth or second molar dies and the patient gets a toothache which feels like wisdom tooth pain .

 Most wisdom teeth are so far back in the mouth that they can't be cleaned. When a patient opens the cheeks press tightly against all four wisdom teeth and the tight cheek prevents a toothbrush from cleaning the wisdom teeth. It is not uncommon for the front of a wisdom tooth to decay and cause wisdom tooth pain especially with hot and cold foods. Because of their space problems, many wisdom teeth are twisted and turned into very unusual positions. The positioning problems prevent these teeth from being cleaned naturally and they often develop cavities .

Bone and tissue impaction
Each individual wisdom tooth is unique and depending on how it grows, it can have different impact on bones and/or tissues. The following 4 impactions are commonly found in patients cases.

If the impaction is between the third molar and part of the second molar, this kind is called bone impaction. It can subcategorized into vertical, horizontal, and angular depending on the way they are intersecting.

  • Bone Impaction (Vertical)
    Impaction would cause pathosis inside the cheek bone or jaw bone.
  • Bone Impaction (Horizontal)
    Besides leading pathosis, horizontal placement would hurt the hard tissue of the second molar to cause toothache and cavities.
  • Bone Impaction (Angular)
    Besides leading pathosis, angular placement would hurt the hard tissue of the second molar to cause toothache and cavities.

The last one is called tissue impaction, because the wisdom tooth is directly breaking out of the tissue.

  • Tissue ImpactionTissue Impaction
    The wisdom tooth almost erupts out of gum, or its half erupts out of gum to cause pseudo-pocket. Besides leading pathosis, it easily tracts food debris and causes gum irritation.

Only under the X-ray can these impactions be clearly identified.

 
Extraction of impacted tooth
The following shows how to remove the impacted wisdom tooth (bone impaction).

     

1. Locate the impacted wisdom tooth. (left)
2. Incise soft tissue to open. (middle)
3. Remove overlying bone to expose the tooth crown. (middle)
4. Extract the tooth out by whole or in pieces. (middle)
5. Suture the soft tissue back. (right)

This procedure is only needed when your wisdom teeth start to bother you, or upon your dentist recommendation.
